Yes, three people can play American mahjong. The usual method is to remove one wind from play and deal only three hands, with the Charleston shortened because there is no fourth player to pass to. The official National Mah Jongg League rules assume four players, so three-handed play always involves house rules.
Being one player short is the most common reason a mahjong set stays in its box. It does not have to be.
How Do Three People Play Mahjong?
Two approaches are common, and groups tend to settle on one.
Remove a wind
Take one wind out of play, usually North, and deal three hands. The wall is built the same way. Hands on the card requiring the removed wind become unplayable, which narrows the options but keeps the game recognizable.
Play a dummy hand
Deal four hands as normal, leaving the fourth face down and unplayed. The wall is the correct size, all tiles stay in circulation, and every hand on the card remains possible. The tradeoff is that a quarter of the tiles are locked away where nobody can reach them, so hands take longer to complete.
The dummy hand method keeps the card fully playable. The removed wind method keeps the game moving faster. Groups that play three-handed regularly usually prefer the dummy hand, because it does not change which hands are available.
What Happens to the Charleston?
The Charleston is built around four players passing in a rotation, so it has to change. Most three-player groups run a shortened Charleston: pass right, across, and left as usual but with only three participants, meaning each pass reaches a different player than it would with four. Some groups skip the second Charleston entirely.
Agree this before dealing. It is the part of three-handed play most likely to cause mid-game disagreement.
Can Two People Play?
Yes, though it is a substantially different game. Two-handed mahjong, sometimes called Siamese mahjong, usually has each player managing two hands. It works, and experienced players use it to practise, but it is not a good introduction for someone learning.
Do I Still Need Four Racks?
Yes, if you are playing the dummy hand method, because the fourth hand still needs somewhere to sit. Even with the removed wind method, the fourth rack is useful for holding the discard pile tidily.

Is Three-Handed Play Worse?
Different rather than worse. Three-handed games are faster, and with fewer players competing for the same tiles, hands complete more often. What is lost is some of the reading of other players, since there is simply less information on the table.
For a group that plays regularly and is occasionally short, it is a good substitute. For learning the game, four players is better, because the Charleston and the flow of discards are easier to understand as designed.
